Job summary

Professional Case Management is hiring RN-Case Managers to provide in-home nursing visits in Washington. You will conduct weekly visits, perform vitals, assess home safety, and coordinate care for former Nuclear Weapons Workers with chronic illness.

Work on a flexible schedule with 1–2 hour visits. Build one-on-one patient relationships while ensuring quality home care, timely documentation, and safety, with benefits including PTO and health coverage.

Qualifications

• Graduate of a state approved school of professional registered nursing.
• Current CPR certification is required.
• Minimum of two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with regional director and staff to identify and improve services.
• Plan, implement, and evaluate care provided; participate in client care conferences as needed.
• Serve as on-site clinical resource and support to ensure client home care needs are met.
