Professional Case Management in Yakima, WA is seeking an RN-Case Manager to perform in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illness. Visits are weekly and typically 1–2 hours, including vitals, home safety checks, and medication planning.

You will provide direct care, develop care plans, and oversee caregivers while coordinating with regional leadership to ensure quality services. Competitive pay and benefits accompany a flexible schedule.

Qualifications

• Must hold a current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification required.

Responsibilities

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate care; coordinate client care conferences as needed.
• Serve as on-site clinical resource and ensure home care needs are met.
• Collaborate with staffing coordinators on personnel scheduling and scope of practice.
