Professional Case Management is seeking RN-Case Managers in New Mexico to deliver in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illnesses. You will assess health, ensure home safety, and coordinate care with caregivers.

Qualifications

• RN license in the state of practice is required.
• Two years of nursing experience with at least one year in home care.
• One year of supervisory/case management experience preferred.
• Current CPR certification is required.

Responsibilities

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate care; participate in care conferences as needed.
• Coordinate with regional director and staff to improve services.
• Oversee staffing and scheduling within scope of practice.
