Qualifications

• Minimum of two years nursing experience, including one year in home care.
• Current, unrestricted RN license in the state(s) of practice.
• Experience in supervisory or case-management roles is preferred.

Responsibilities

• Responsible for direct care and case management of assigned clients.
• Assist and collaborate with the regional director to improve services.
• Plan, implement, and evaluate care provided.
