Job summary

Professional Case Management (PCM) seeks an RN Case Manager to conduct in-home nursing visits for former nuclear weapons workers in Washington. You will assess vitals, ensure home safety, manage medications, and oversee caregivers weekly, with visits typically lasting 1–2 hours.

Required include RN license in practice state, 2+ years nursing experience (1 year in home care), and CPR. BSN preferred; benefits include PTO and 401(k).

Qualifications

• Graduate of a state approved school of professional registered nursing.
• BSN preferred.
• Current, unrestricted RN license in the state(s) of practice.
• Minimum of two (2) years nursing experience including one (1) year in home care or closely related field.
• One (1) year of supervisory and/or case management experience preferred.
• Current CPR certification.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided.
• Coordinate and manage client care conferences as needed.
• Serve as a local on-site clinical resource and support to meet home care needs.
• Assist staffing coordinators regarding staffing within scope of practice and client needs.
• Adhere to nursing delegation guidelines and agency policies.
• Participate in performance improvement initiatives and clinical documentation reviews.
