Job summary

Professional Case Management is seeking in-home RN Case Managers in Washington. You will conduct weekly in-home nursing visits for former Nuclear Weapons Workers with chronic/terminal illness, performing vitals, home safety checks, and medication planning.

Qualifications

• RN license in state(s) of practice.
• BSN preferred.
• Minimum of two years nursing experience including one year in home care or closely related field.
• One year of supervisory and/or case management experience preferred.
• Current CPR certification.
• Graduate of a state approved school of professional registered nursing.

Responsibilities

• Provide direct care and case management of assigned clients.
• Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services.
• Plan, implement, and evaluate care provided; participate in client care conferences as needed.
• Coordinate staffing and scheduling within scope of practice and client needs.
• Ensure adherence to Agency policies and nursing delegation guidelines.
• Participate in performance improvement initiatives and clinical documentation reviews.
