Job summary

Professional Case Management seeks RN-case managers to provide in-home care to former Nuclear Weapons Workers with chronic/terminal illness in South Carolina. You will perform direct nursing care, case management, and assessments, coordinating with the regional director and care staff.

Build strong patient relationships and develop comprehensive care plans that fit a flexible schedule. Requirements include an active RN license, BSN preferred, and at least two years nursing experience with home

Qualifications

• Must have active RN license in state of practice.
• BSN preferred but not required for all cases.
• Minimum 2 years nursing experience including 1 year in home care.
• 1 year supervisory or case management experience preferred.
• Current CPR certification is required.

Responsibilities

• Provide direct care and case management for assigned clients.
• Plan, implement, and evaluate care provided; participate in client care conferences.
• Coordinate with regional director and other personnel to improve services.
• Assist with staffing coordination and scheduling within scope of practice.
• Review clinical documentation for accuracy and completeness.
• Participate in performance improvement initiatives and reviews.
