Job summary

Professional Case Management (PCM) is seeking a dedicated RN-Case Manager to conduct in-home nursing visits for former nuclear workers in Georgia. You will perform vitals, assess home safety, coordinate care, and supervise caregivers to ensure high-quality patient outcomes.

The role offers a flexible schedule, a $42/hour rate, weekly pay, and comprehensive benefits. You will work with a regional director to ensure adherence to policies and continuous improvement of care.

Qualifications

• RN case management experience in home care.
• Two years nursing experience with one year in home care.
• BSN preferred; active, unrestricted RN license.

Responsibilities

• Provide direct care and case management of assigned clients.
• Perform nursing assessments and monitor vitals and safety.
• Coordinate with regional director and care staff to ensure quality services.
• Plan and evaluate care provided and participate in care conferences.
