Job summary

Institute on Aging in California is seeking a Temporary, full-time Care Manager/Registered Nurse (RN) to support the HCBA Medi-Cal Waiver program. You will conduct comprehensive nursing assessments, coordinate care, and develop POTs for participants across Riverside and San Bernardino Counties.

This role requires an active CA RN license, BLS, and willingness to travel. You will work with a multidisciplinary team, document care plans in MedCompass, and monitor waiver services to ensure timely,

Qualifications

Current, unencumbered RN license in California.
BLS certification is required.
Minimum 1000 hours in the past two years in an acute care hospital with relevant care levels.
Valid driver's license and proof of insurance.
Willingness to travel in Riverside and/or San Bernardino Counties.
Minimum 2000 hours in the past five years in related settings for DHCS waiver care.
Experience establishing goals for patient care and continuity of care.
Demonstrated ability to coordinate care and advocate for participants.

Responsibilities

Conduct comprehensive nursing health assessments and determine required services.
Coordinate care with participants, their representatives, and primary care physicians.
Develop Plan of Treatment (POT) and coordinate service availability.
Identify backup caregivers and arrange for unpaid support when needed.
Provide information, education, counseling, and advocacy to participants.
Establish and monitor care coordination schedules based on needs and reassessments.
Monitor Waiver services to ensure services match POT authorizations.
Maintain ongoing contact with participants via visits or calls for changes in health or well-being.
Perform annual reassessments and care plan updates.
