Job summary

Complete Home Care, Inc. seeks a licensed RN to administer nursing care in clients' homes, coordinating with the care team, clients, families, and referring agencies.

You will complete assessments, develop and revise the Plan of Care, and monitor outcomes to ensure quality and continuity. Strong communication, critical thinking, and infection control are essential; travel to local clients is required.

Qualifications

• Completion of an accredited Registered Nursing Program.
• One year RN experience in clinical care; home health preferred.
• Current CPR certification and valid RN license in the state of practice.

Responsibilities

• Administer nursing care in patients' homes and coordinate with the interdisciplinary team.
• Complete initial and ongoing assessments and identify patient needs, including OASIS.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity.
• Provide effective communication to patients, families and health professionals; participate in case conferences.
• Supervise, teach and provide direction to other nursing personnel; perform on-call duties.
• Document timely to comply with company policy and maintain competency.
