Job summary

Home & Health Care Management, Inc. is seeking a Registered Nurse to join an interdisciplinary care management team in a home-based care setting in Redding, CA.

You will supervise care for clients receiving home and community services, review plans, communicate with providers, and coordinate with case managers. The role requires an active California RN license, a driver’s license, a clean DMV record, your own vehicle, and CPR within 30 days.

Qualifications

• Active California RN license required.
• 1+ year RN work experience preferred.
• Proficiency with software programs; MS Office (Word, Excel) required.
• CPR certification within 30 days of employment.

Responsibilities

• Review Medical Records including Plan of Treatment and services to ensure appropriate care.
• Communicate with providers, patients, caregivers, HUD housing representatives in verbal and written formats; document interactions in EHR.
• Work with interdisciplinary care team; coordinate with Case Management RN and Social Work staff.
