Job summary

FreedomCare in New Haven, CT seeks an RN Home Health Case Manager to conduct initial in-home assessments and ongoing care coordination. You will develop and implement patient care plans, collaborate with physicians and interdisciplinary teams, and ensure timely initiation of services.

Qualifications

• Diploma from an accredited nursing program.
• RN license in Connecticut.
• Care coordination experience is a plus.

Responsibilities

• Conduct initial in-person clinical assessments and coordinate service activation.
• Develop and manage patient’s plan of care; revise as needed.
• Coordinate care with physicians and other disciplines; document changes.
• Provide ongoing coaching to drive best practices and caregiver education.
• Review caregiver notes and adapt to changes in condition or environment.
• Ensure compliance with regulatory standards and company policies.
• Perform other duties as directed by supervisor.
