Job summary

FreedomCare is seeking a Care Manager, RN in Connecticut to conduct in-home assessments, orient new patients and caregivers, and provide ongoing care management. The role combines clinical assessment, care coordination, education, and quality assurance with regular home visits.

Responsibilities include developing care plans, coordinating with physicians and teams, and ensuring regulatory compliance while delivering person-centered care. New graduates welcome to apply, with travel as needed.

Qualifications

• Diploma from an accredited nursing program.
• New graduates welcome; Home care case management is a plus.
• Active RN license in Connecticut.
• Ability to travel within assigned geographic area; valid driver’s license and car insurance.

Responsibilities

• Conducts initial in-person clinical assessments and coordinates service activation following procedures.
• Develops, implements, and manages patient’s plan of care.
• Revisions and documents changes as needed.
• Coordinates care with physicians and other disciplines involved in the client’s care and community resources.
• Provides ongoing coaching to drive effective utilization of best practices and caregiver education.
• Reviews caregiver notes daily and addresses changes in patient’s condition.
• Ensures compliance with regulatory standards and company policies.
• Performs other duties as directed by supervisor.
