Job summary

Health Wealth Safe, Inc. is seeking an in-home nursing case manager to deliver comprehensive case management for patients enrolled in the Advanced Care at Home program.

The role focuses on assessing clinical status, coordinating care plans, and connecting patients with community resources in a home-based setting. You will monitor treatment progress, escalate concerns as needed, and educate patients and families on care plans and self-management while maintaining clear communication with the care
