Job summary

Institute on Aging seeks a Temporary, full-time Care Manager/Registered Nurse (RN) to support the HCBA Medi-Cal Waiver program across Riverside and San Bernardino Counties. You will perform nursing assessments, coordinate care, and work with a multidisciplinary team to meet participants' community-based needs.

This position requires driving, flexible scheduling, and the ability to develop care plans, monitor services, and communicate with participants and families.

Qualifications

• Current, unencumbered license to practice as an RN in the State of California.
• Current Basic Life Support (BLS) certification.
• A valid driver’s license and proof of insurance.
• Must be willing to travel in Riverside and/or San Bernardino Counties.

Responsibilities

• Document assessment, case notes, care coordination, and utilization management in MedCompass.
• Work with participants, their legal representatives, circle of support, and primary care physicians.
• Develop goals associated with assessed needs, individual circumstances, and preferences.
• Develop a Plan of Treatment (POT) to mitigate risk and minimize disruptions in services.
• Identify and coordinate backup caregivers when waiver services are delayed.
• Provide information, education, counseling, and advocacy to participants.
• Establish a care coordination schedule based on needs and acuity.
• Monitor delivery of HCBA Waiver services and participants’ well-being.
• Conduct annual visits, reassessments, and care plan updates.
