Job summary

Institute on Aging Southern California, LLC (IOA) seeks a Temporary, full-time Care Manager/Registered Nurse to support the HCBA Medi-Cal Waiver program. The role focuses on care coordination in the community, working with participants, families, and clinicians to implement and monitor plans of care across Riverside and San Bernardino Counties.

Qualified candidates hold CA RN license, BLS, and substantial recent acute care and home health experience, with strong communication and documentation

Qualifications

• Current, unencumbered license to practice as an RN in the State of California.
• Current Basic Life Support (BLS) certification.
• Minimum 1000 hours of experience in the previous two years, in an acute care hospital caring for individuals with the care needs specified in this waiver.
• Valid driver's license and proof of insurance.
• Must be willing to travel in Riverside and/or San Bernardino Counties or a minimum of 2000 hours in the previous three years in an acute care hospital caring for individuals with the care needs specified in this waiver.
• Minimum of 2000 hours in the previous five years working for a licensed and certified home health agency caring for individuals with the care needs specified in this waiver.
• Experience establishing short and long-range goals for patient care within nursing process.
• Experience with medical diagnoses and medication for functionally impaired adults and older adults.

Responsibilities

• Document assessment, case notes, care coordination, and utilization management in MedCompass.
• Collaborate with participants, their representatives, circle of support, and primary care physicians to set goals.
• Develop a Plan of Treatment (POT) to mitigate risk and ensure continuity of services.
• Identify services available through community resources and public programs.
• Implement the POT and coordinate providers and resources for timely delivery.
• Identify and train backup caregivers for unpaid support when waiver providers are unavailable.
• Provide information, education, counseling, and advocacy for participants.
• Establish a care coordination schedule based on initial assessment and reassessments.
• Monitor Waiver services for compliance with POTs and quality standards.
• Maintain regular contact with participants (monthly) to monitor health, mood, social integration, and functioning.
• Conduct annual visits and reassessments; update care plans as needed.
• Follow up after emergency department visits or inpatient care.
• Perform additional duties as assigned.
