Job summary

Monogram Health in Fort Oglethorpe, GA is seeking a Registered Nurse Care Manager to partner with an integrated care team. You will conduct in-home visits, develop and adapt care plans, and support CKD/ESRD patients to improve quality of life.

You will work with physicians, pharmacists, and social workers, coordinate with dialysis providers, educate patients and families, and travel locally to patient homes.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license for the posting location (GA).
• Minimum 2+ years in care management or CKD/ESRD care.
• BLS certification required, employer supports certification.
• Willingness to travel locally for in-home visits.

Responsibilities

• Develop and continually adapt patient care plans with clinicians.
• Perform in-home care management visits and execute plans.
• Monitor biometric data and intervene per protocols.
• Coordinate meds with pharmacists and prescribers.
• Educate patients and families on CKD/ESRD, dialysis, and health habits.
• Engage family and social support groups in care.
• Be primary contact for patients during business hours.
• Educate on medications, diet, exercise, and lifestyle.
• Support advance care planning and end-of-life discussions.
• Document updates in care management platform.
• Coordinate transitions with dialysis providers.
