Job summary

Monogram Health in Georgia is seeking a Care Manager RN to support in-home care for CKD/ESRD patients, working with a small panel to improve quality of life at home. You will coordinate with physicians and other clinicians to implement and adjust care plans.

The role requires a Georgia RN license, BLS certification, 2+ years in care management, and willingness to travel locally; some travel to Brentwood, TN may be needed for training, with on-call nights possible.

Qualifications

• Graduate of an accredited School of Nursing.
• Licensed RN in the State of Georgia.
• 2+ years in care management or CKD/ESRD care.
• BLS certification required; company supports certification onboarding.
• Willingness for local travel and some domestic travel for training.

Responsibilities

• Develop and adapt care plans with patients’ medical providers.
• Perform in-home care management visits and follow care plans.
• Monitor biometric data and follow protocols for interventions.
• Coordinate medications with pharmacists and prescribers.
• Educate patients on CKD/ESRD, dialysis, and related comorbidities.
• Encourage adherence and engage families and social support groups.
• Serve as primary contact during business hours for patients.
• Support Advance Care Plans and proactive care discussions.
• Document updates in the care management platform and coordinate with dialysis providers.
