The Legacy Senior Communities seeks an experienced RN to provide skilled nursing care in a home health setting under the patient plan of care. You will assess, educate, and coordinate services to promote optimal outcomes for the client.

You will conduct admissions, document thoroughly, and participate in discharge planning, while collaborating with physicians and other clinicians. Current Texas RN license and CPR are required.

Qualifications

• Graduation from an accredited School of Nursing.
• Current license in the state of Texas.
• Current CPR certification.
• One or more years in a home health agency or hospital; home health experience preferred.

Responsabilités

• Conduct admission of the patient when necessary.
• Obtain medical history related to the present condition from patient and/or family.
• Perform comprehensive assessment including OASIS and home safety evaluation for teaching needs.
• Develop and update the plan of care with the physician and clinicians.
• Participate in discharge planning for the patient.
• Provide skilled nursing care per the plan of care.
• Prepare and present patient records at case conferences as needed.
• Coordinate with other agencies to ensure continuity of care.
• Pursue in-service education and professional development to improve nursing care.
• Maintain clinical records: visit notes, assessments, and medication profiles within required timelines.
• Maintain confidentiality of patients' health information.
