Qualifications

• Active Texas RN license.
• 2+ years of Home Health RN experience.
• Current CPR certification.
• Reliable transportation and valid driver licence.

Responsabilités

• Case management and supervised visits.
• Perform skilled and comprehensive assessment of the patient during the initial admission.
• Assess and chart observations of the patients condition at each visit (vital signs).
• Provide skilled nursing care as needed per the plan of care (medication administration, wound care, healing progress).
• Provide proper patient education for home health procedures and strategies.
• Maintain thorough clinical records, visit notes within 24 hours, OASIS within 48 hours, med profiles and orders.
• Maintain confidentiality of patient PHI under HIPAA.
• Provide comfort through palliative care.
