Job summary

Qualifications

• RN must be currently licensed and in good standing.
• One year of RN experience in a clinical setting preferred Home Health experience.
• CPR certification required and maintained.
• Must read, write, and comprehend English.

Responsibilities

• Promote Lifecare Home Health mission and values.
• Complete initial and ongoing home health assessments including OASIS where applicable.
• Develop, initiate, and revise the Plan of Care to ensure quality and continuity of care.
• Coordinate services and communicate changes to physicians and team members.
• Manage documentation timely to ensure compliance with policies.
• Provide supervision and guidance to other nursing personnel as assigned.
• Cover on-call duties as required.
• Maintain continuing education and competency.
