Job summary

HHN Healthcare is seeking a Registered Nurse to administer skilled nursing care in patients' homes. The role includes coordinating with an interdisciplinary team, patient families, and referring agencies, and managing care plans to meet patient needs.

The RN will perform comprehensive home health assessments, implement and revise the plan of care, and provide skilled nursing services while ensuring infection control and safety. A Texas RN license and a valid driver’s license are required.

Qualifications

• Current Texas State license as a Registered Nurse (RN).
• CPR certification and TB/Hepatitis status as applicable.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Perform initial and ongoing comprehensive home health assessments (including OASIS at appropriate times).
• Develop and revise the plan of care as needed; initiate preventive and rehabilitative nursing procedures.
• Provide skilled nursing services with infection control measures and patient safety in mind.
