Job summary

Pathways Healthcare is seeking an experienced registered nurse to join our home health and hospice team in Massachusetts. You will assess patient needs, develop and revise plans of care, and coordinate with physicians to ensure high-quality in-home care.

The role offers a full-time schedule (Monday to Friday) with competitive salary and a comprehensive benefits package. Training is provided, and a vehicle is required for patient visits in the Concord area.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient’s status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
• Initiates appropriate preventive and rehabilitative nursing procedures.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition.
