Qualifications

2+ years' nursing experience, preferably in geriatrics is desirable.
New nurses, feel free to apply! We have a great training program.
Degree in Nursing from an accredited institution.
Proficient with electronic medical records.
Excellent bedside manner and communication skills.
Working knowledge of health care laws and regulations.
Maintain current BLS CPR certification.
Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

Assesses and evaluates patient’s status by writing and initiating plan of care.
Regularly re-evaluating patient and family/caregiver needs.
Participating in revising the plan of care as necessary.
Develops a care plan that establishes goals based on nursing diagnosis and incorporates nursing actions.
Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
Completes, maintains and submits accurate and relevant clinical notes.
Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition.
Provides a safe environment for the patient.
Attends interdisciplinary group meetings.
Participates in on-call duties.
Ensures arrangements for equipment and services are available.
Supervises ancillary personnel and delegates responsibilities when required.
Maintains and upgrades professional knowledge through continuing education.
Fulfills case assignments.
Acts in quality assessment and performance improvement teams.
