Louisiana HomeCare, part of the LHC Group and within the Optum family, seeks a Registered Nurse in Home Health to deliver nursing care at patients' homes and coordinate the overall plan of care with physicians and the care team.

The role emphasizes clinical nursing, documentation, and collaboration with family and providers. A valid LA RN license and reliable transportation are required; some LA-specific experience is preferred.

Competenze

• Current and unrestricted RN licensure in state of practice.
• Driver's license and access to a dependable vehicle or public transportation.
• LA: 1+ years of clinical RN experience (may be waived for an RN with recent experience as an LPN).
• RN licensure must have no restrictions.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• 1+ years of Home Health experience.

Mansioni

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks per regulations.
• Provides supervisory visits and ensures high-quality care and infection control standards.
