Job summary

Louisiana HomeCare, part of the LHC Group and aligned with the Optum family, seeks an RN for home health. The role involves initiating and updating the care plan with physicians and supervising aides.

You will perform OASIS assessments, ensure medication reconciliation, and maintain high-quality documentation in coordination with the care team. The position requires current RN licensure in Louisiana, a valid driver's license, and the ability to work within state guidelines, including supervision

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• LA state requirements: 1+ year RN clinical experience; RN licensure must have no restrictions.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses; provides instruction and assigns tasks per regulations.
• Provides high-quality clinical services within scope of practice and infection control standards; completes comprehensive assessments (OASIS) including medication reconciliation.
• Communicates with physicians, patients, and family to ensure quality care and timely documentation.
