Job summary

Louisiana HomeCare of Raceland, part of LHC Group and UnitedHealth Group family, seeks an LPN for home health care to provide skilled nursing in patients' homes. Responsibilities include delivering care per the plan of care, documenting visits in the EMR, and coordinating with the care team.

Required: active LA LPN licensure, valid driver's license, and reliable transportation. Preferred: CPR certification, home care experience, and strong communication skills.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and in coordination with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory requirements.
• Participates in the agency's care coordination, communicating needs and changes in condition to the patient, family, and clinicians.
• Function effectively in any home situation regardless of client demographics.
