Job summary

Louisiana HomeCare, part of LHC Group and a member of the Optum family, is seeking a Registered Nurse in Home Health to provide and direct nursing care in patients' homes as prescribed by physicians and in accordance with agency policies.

You will coordinate the plan of care with other healthcare professionals, supervise home health aides, complete comprehensive assessments, and ensure documentation compliance.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• LA-specific requirements: RN licensure with no restrictions and 1+ years of clinical RN experience.
• Must have a valid driver's license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Direct nursing care in patients' homes as prescribed by the physician and in accordance with laws and agency policies.
• Coordinate total plan of care with other health care professionals and ensure continuity of patient care.
• Supervise home health aides and licensed nurses; complete required supervisory visits and documentation.
• Complete assessments (OASIS), medication reconciliation, and timely charting per policy and payer requirements.
