Job summary

Louisiana HomeCare Egan, a part of LHC Group and Optum family, is hiring a Registered Nurse in Home Health to provide and direct nursing care to patients in their homes as prescribed by physicians and policies.

You will coordinate care plans, supervise home health aides, complete OASIS assessments, ensure compliance, and collaborate with physicians, families, and care teams to maintain continuity of care and high-quality outcomes.

Qualifications

• Current and unrestricted RN license in LA state.
• Valid driver's license, vehicle insurance, and access to a reliable vehicle.
• CPR certification preferred or ability to obtain within 90 days.

Responsibilities

• Initiates and revises the plan of care in collaboration with physicians and health team.
• Supervises home health aides and licensed nurses, assigns tasks per regulations.
• Performs comprehensive assessments (OASIS) and medication reconciliation.
• Documents visits and communicates effectively with patients, families and clinicians.
