Job summary

Louisiana HomeCare is seeking a Registered Nurse for Home Health with a valid LA license and at least 1 year of clinical RN experience. The RN will plan and deliver patient care in home settings, supervise aides, and coordinate with physicians and families to ensure high-quality outcomes.

The role includes performing OASIS assessments, maintaining documentation per policy, and participating in on-call rotations. A comprehensive benefits package is provided.

Qualifications

• RN licensure in state of practice (LA)
• Valid driver’s license and access to a dependable vehicle
• 1+ year of RN clinical experience in Louisiana
• RN licensure must have no restrictions in LA if specified

Responsibilities

• Initiates and revises the plan of care with physician and team
• Supervises home health aides and LPN/LVN nurses per regulations
• Completes OASIS assessments and medication reconciliation
• Documents visits and maintains policy-compliant records
• Communicates with physicians, patients and families to ensure quality care
• Participates in on-call and weekend rotations
• Contributes to survey readiness and performance improvement
