CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to support patients in their homes under the supervision of a Physical Therapist. You will administer care to improve strength, mobility and independence for clients in a home setting.

This role requires a valid PTA license, CPR, and a valid driver’s license, with the ability to work with an aging population. The position offers pay-per-visit rates with full-time hours and patient-facing responsibilities.

Qualifications

• Must hold a valid Physical Therapy Assistant license in the state.
• CPR certification is required.
• Experience collaborating with healthcare professionals is preferred.
• Strong understanding of geriatric physical, emotional, social, and biological changes.

Responsabilités

• Administer the PT treatment plan in patients' homes as developed by the PT.
• Perform non-complex manual therapy and modalities.
• Document patient records and submit weekly to the office.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment failures or staffing changes.
• Apply infection control and universal precautions.
