Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to support patients in their homes under the guidance of a Physical Therapist. You will deliver treatment plans, monitor progress, and coordinate with the care team to improve functional outcomes.

Qualified candidates have a state PT license, CPR certification, a valid driver's license, and the ability to work with geriatric patients. English fluency is required; one year of PTA experience preferred.

Qualifications

• Current license to practice Physical Therapy in the state.
• Valid driver’s license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of any equipment failures, lack of supplies, or staff or client changes.
• Apply concepts of infection control and universal precautions in performing patient care activities.
