Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist Assistant to deliver PT interventions in patients' homes under the supervision of a Physical Therapist. You will administer treatment plans, document progress and coordinate with the care team to support functional improvements and independence.

The role requires a PTA license, valid driver's license, CPR, and strong teamwork, with an emphasis on geriatric care and infection control.

Qualifications

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Knowledge of geriatric physical, emotional, social and biological changes.
Read, write and speak fluent English.

Responsibilities

Administer the physical therapy treatment plan to patients, working onsite in their homes, as developed by the Physical Therapist.
Administer non‑complex active and passive manual therapies and modalities to patients.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in‑services.
Notify Physical Therapist or Branch Director of equipment issues or staff or client changes.
Apply infection control and universal precautions in patient care.
