Job summary

CenterWell Home Health is seeking a dedicated Home Health Physical Therapist Assistant to help patients regain strength and independence in their own homes. You will work under the supervision of a Physical Therapist and support a multidisciplinary team to deliver patient-centered care.

The role requires an active license, valid driver’s license, CPR certification, and strong teamwork, with a focus on geriatric health and functional improvement.

Qualifications

• Current state license to practice Physical Therapy or PTA license
• Valid driver’s license, auto insurance and reliable transportation
• Current CPR certification
• Experience collaborating with a healthcare team
• Comprehensive knowledge of geriatric health considerations
• Read/write/speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes.
• Deliver non-complex manual therapies, therapeutic exercises, and modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment, supply, or staff changes.
• Apply infection control and universal precautions in patient care.
