Job summary

TheraCare in San Antonio, TX is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health role providing in-home therapy to adults and seniors.

You will work under a supervising PT, schedule-flexible hours, and deliver one-on-one interventions designed to improve mobility, safety, and independence in patients' homes. This position emphasizes autonomy and clinical judgment with competitive benefits.

Qualifications

• Associate degree from an accredited PTA program.
• Active PTA license in Texas.
• Home Health experience (1 year required).
• HomeCareHomeBase (HCHB) experience preferred.
• Current CPR certification.
• Strong communication, clinical judgment, and time-management skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Provide skilled home health PTA interventions under supervision of a PT.
• Carry out individualized Plans of Care (POC) delegated by the supervising PT.
• Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions.
• Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities.
• Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments.
• Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies.
• Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist.
• Complete accurate and timely electronic documentation in compliance with Medicare home health regulations.
• Follow infection control standards and home health safety protocols.
• Participate in QAPI and agency performance improvement initiatives.
