Job summary

TheraCare is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health role in San Antonio, TX. You will provide one-on-one in-home PT services to adults and seniors, creating your own schedule while promoting mobility, strength, safety, and independence in patients' homes.

Responsibilities include implementing POC, patient education, documentation, and participating in quality initiatives. Texas PTA license and PTA degree required; Home Health experience preferred.

Qualifications

• Associate degree from an accredited PTA program.
• Active PTA license in Texas.
• CPR certification and current license.
• HomeHealth/Homecare experience preferred.

Responsibilities

• Provide skilled home health PTA interventions under supervision of a licensed PT.
• Implement and carry out Plans of Care (POC) developed by the supervising PT.
• Treat adults and seniors with orthopedic, neurological, cardiac, and post-surgical conditions.
• Administer therapeutic exercises, gait and balance training, and functional mobility activities.
• Apply manual therapy techniques and modalities as ordered.
• Educate patients and caregivers on home exercise programs and safety.
• Document accurately and timely in compliance with Medicare home health regulations.
• Follow infection control and home health safety protocols.
• Participate in QAPI and performance improvement initiatives.
