Job summary

Pathways Healthcare in Watertown, MA, is seeking a Physical Therapist (PT) to provide in-home patient care as part of our physician- and nurse practitioner-led team. This role involves evaluating patients, formulating rehabilitation plans, and coordinating care with families to promote recovery at home with dignity.

You will assess functional status, develop individualized PT programs, train caregivers, and participate in discharge planning to ensure seamless transitions to community services.

Qualifications

• Degree as a physical therapist by an accredited organization.
• Minimum of two years of experience as a physical therapist assistant; home/community health experience preferred.
• Excellent verbal and written communication skills and strong organizational abilities.
• BLS CPR certification current.

Responsibilities

• Provide physical therapy services per physician's plan of care and state law.
• Develop individualized PT programs and train family members involved in care.
• Instruct patients and families in use of therapeutic devices and prosthetics; provide in-service education.
