Job summary

Empower Home Health - a LiveWell Partners Network Agency is seeking a Physical Therapy Assistant (PTA) to deliver PT services under supervision in home health settings. You will work with a diverse care team to support patient goals and ensure continuity of care.

Responsibilities include assisting PTs, documenting notes, educating patients and caregivers, and coordinating scheduling with the care team. The role emphasizes in-home treatment plans and collaborative patient care.

Responsibilities

• Assist the physical therapist in following physical therapy care plans.
• Prepares clinical and progress notes and helps develop the treatment plan (revising as necessary).
• The Physical Therapy Assistant (PTA) performs PT services in line with the care plan and physician orders.
• Consult with the physical therapist regarding changes in treatment.
• Instruct patients and caregivers on in-home programs and activities of daily living.
• Participate in interdisciplinary case conferences to contribute to patient updates, discharge planning, etc.
• Advises and consults with agency personnel and patient family/caregivers.
• The Physical Therapy Assistant (PTA) confirms on a weekly basis the scheduling of visits with the supervising physical therapist, care team, office staff, etc.
