Qualifications

• Hold a degree from an accredited Physical Therapy program.
• Valid license to practice as a Physical Therapist in the state.
• Maintain current BLS CPR certification.

Responsibilities

• Deliver physical therapy services per physician's plan of care and state laws.
• Assess rehabilitation status and home environment.
• Perform muscle strength, mobility, gait, and ROM assessments.
• Administer treatments and instruct on use of devices.
• Report treatment responses and condition changes to physician.
• Initiate and manage PT programs in collaboration with care team.
• Instruct family and caregivers on therapy goals.
• Provide training on prosthetic devices and refer for additional services.
• Contribute to discharge planning and case conferences.
