Job summary

Green Meadows Home Health Care Inc is seeking a full-time Physical Therapist Assistant (PTA) to join an interdisciplinary home health team in California. You will work under the supervision of a licensed Physical Therapist to help implement rehab plans at patients' homes.

You will assist with exercises, monitor progress, document changes, and educate patients and caregivers on safe techniques. Travel within the designated service area is required, with flexibility in hours.

Qualifications

• Associate’s degree from an accredited PTA program.
• Active PTA license in the state of practice (CA).
• Current CPR/BLS certification.
• 1–2 years PTA clinical experience; home health preferred.

Responsibilities

• Implement treatment plans under supervision of a licensed PT.
• Provide hands-on assistance with prescribed exercises.
• Assist with ADLs, mobility, and balancing activities.
• Monitor progress and document changes in condition.
• Educate patients and caregivers on exercises and safety.
• Coordinate with family and healthcare team for home care.
• Complete accurate documentation per policy and CMS guidelines.
