Job summary

Omega Home Health Care Services, Inc. is seeking a dedicated Home Health Physical Therapy Assistant (PTA) to work under a licensed Physical Therapist to provide rehabilitative services in patients' homes. This role offers flexible scheduling and per-visit compensation, serving diverse communities across California.

The PTA must be licensed in California, capable of independent work, and possess strong communication and driving skills to coordinate care and support patients in their homes.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current California PTA license in good standing.
• Minimum of one year of home health experience preferred.
• Strong communication and organizational skills.
• Ability to work independently in a home-based setting.

Responsibilities

• Deliver physical therapy treatments per supervising PT.
• Monitor and document patients' progress and responses to treatment.
• Educate patients and families on exercises and mobility techniques.
• Collaborate with interdisciplinary team to coordinate patient care.
• Adhere to state, federal, and agency regulations and protocols.
• Maintain accurate and timely clinical documentation.
