Job summary

Green Meadows Home Health Care Inc. is seeking a full-time Physical Therapist Assistant (PTA) to work in patients' homes under the supervision of a licensed Physical Therapist. The role focuses on implementing care plans, guiding exercises, and promoting functional independence.

Expect collaboration with a multidisciplinary team, travel within service areas, and strong documentation practices. A PTA license in California, CPR/BLS, and 1–2 years of PTA experience are preferred.

Qualifications

• Active PTA license in California (state of practice).
• CPR/BLS certification.
• Minimum 1–2 years clinical PTA experience; home health preferred.

Responsibilities

• Implement treatment plans developed by supervising PT, including therapeutic exercises and mobility training.
• Assist patients with prescribed exercises and ADLs, ensuring safety and proper technique.
• Monitor patient progress and document changes in condition; adjust care as advised by PT.
• Educate patients and caregivers on exercise techniques, safety, and fall prevention.
• Collaborate with supervising PT and health-care team to ensure care plan adherence.
• Complete accurate documentation of visits and progress in accordance with policies.
