Job summary

Qualifications

Responsibilities

• Implement physical therapy services per the plan of care.
• Provide therapeutic exercises and interventions to improve mobility and independence.
• Educate patients and caregivers on exercises, safety and fall prevention.
• Document patient responses and progress accurately and timely.
• Coordinate care with supervising PT and clinical leadership.
• Maintain compliant clinical notes and progress reports.
