Job summary

Qualifications

• Graduate from CAPTE-accredited PTA program.
• Current PTA license in the assigned state.
• Current CPR certification; minimum 1 year home health PTA experience.

Responsibilities

• Provides physical therapy services under the supervision of a licensed PT in patients' homes.
• Monitors and documents patient response to treatment and reports changes to the supervising PT.
• Maintains confidentiality and documents supervisory activities in the medical record.
• Supports care coordination and referrals to community resources as needed.
