Job summary

Select Home Health is seeking a licensed Physical Therapist Assistant (PTA) to provide in-home therapy under the supervision of a Registered PT. The role delivers direct patient care per an established plan of care, monitors progress, and supports patients and caregivers to achieve safe, functional independence at home.

Requirements include a CAPTE-accredited PTA program, passing the PTA exam, Indiana PTA license, and a minimum of 2 years PTA experience (home health preferred).

Qualifications

• CAPTE-accredited Physical Therapist Assistant program completed.
• Indiana PTA license active and in good standing.
• Minimum 2 years PTA experience; home health experience preferred.

Responsibilities

• Provide in-home PT interventions per physician-approved plan of care.
• Monitor patient progress and report changes to PT/DOR/DON.
• Educate patients and caregivers on exercises and safety precautions.
• Document accurately and timely per agency policies and regulations.
• Participate in care conferences and interdisciplinary meetings.
