Job summary

FOX Rehabilitation is expanding in Pottsville and surrounding areas and seeks licensed Physical Therapists to deliver one-on-one care to older adults in private residences and senior living communities. Qualifications

• Active Physical Therapy license in the state(s) of practice or eligibility to obtain
• Current CPR certification
• Basic computer and EMR proficiency
• Ability to work independently with patients in home and senior living settings

Responsibilities

• Provide one-on-one physical therapy to older adults in private homes or senior living communities
• Develop individualized plans of care and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and quality of life
• Document in FOX’s EMR system in compliance with regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Offer telehealth/virtual care when appropriate to extend access
