Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated per diem Physical Therapist to provide in-home care in Essex County, NJ. Flexible schedule and a patient-centered approach are encouraged, with both 1099 and W-2 options available.

Pay ranges from $72 to $95 per visit, with opportunities for professional growth and a supportive clinical team. Prior home health experience is preferred, and mentorship is provided for new graduates.

Qualifications

• Licensed Physical Therapist in NJ or eligible.
• Physical Therapist Program Graduate.
• Home health experience preferred but not required - mentorship will be provided.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function and relieve pain.
• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment and adjust to maximize benefit.
• Must be willing to treat geriatric patients.
