Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated per diem Physical Therapist to provide in-home care in Bergen County, NJ (including Teaneck, Bogota, and Paramus). Flexible schedule, 7 days a week availability, and a supportive team environment are offered.

Travel to patient homes is required, with training provided for documentation and compliance. We welcome all levels of experience; new graduates are encouraged to apply.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in New Jersey or eligible.
• Preferred Certifications: First Aid, AED, & CPR.
• Home health experience is preferred but not required - mentoring provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
• Must be willing to treat geriatric patients.

Education

Physical Therapist Program Graduate

Licensed Physical Therapist in New Jersey or eligible
