Job summary

Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, is seeking a motivated part-time Home Care Physical Therapist to provide in-home care in Bergen County, NJ. 20 visits per week with flexible scheduling and a benefits package.

Licensed NJ PT or eligible; new grads welcome with mentorship. Travel to patients’ homes within the geographic territory is required. Competitive compensation and comprehensive benefits are offered.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.
