Job summary

Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, seeks a motivated part-time Home Care Physical Therapist to provide in-home care in Sussex County. 20 visits per week with benefits; flexible schedule. All levels welcome and graduates can train. Travel to patients’ homes required.

Licensed PT in NJ or eligible preferred; mentorship offered for documentation and compliance. Competitive base salary plus benefits and employer-provided growth opportunities.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Preferred certifications: First Aid, AED, CPR.
• Home health experience preferred; mentor provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans.
• Deliver care in line with professional standards and plan of care.
• Educate family members and caregivers on therapy techniques.
• Plan and carry out therapy programs to maintain, improve, or restore function.
• Perform and document an initial exam and diagnose prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate treatment effects and adjust accordingly.
• Willingness to treat geriatric patients.
